As a service to our participants needing a liquid meal replacement we offer Boost Plus. Participants can receive two cases per month. Each case contains 24 cans. The flavors we stock are chocolate, vanilla, and strawberry.
The criteria to participate in the liquid meal
replacement program is as follows:
♦ Be 60 years of age or older.
♦ Have a prescription from a physician, or
♦ Recommendation from a registered dietician |
Individuals will be asked to
fill out a
Request Form and present a doctor's prescription or a
written recommendation from a registered dietician before picking up the
product. Requests and prescriptions can be faxed to our office.
The cost to our participants is by donation for the
first two cases each month.
Cost for each additional case is $ 24.00.
The suggested donation per case is $20.00 or what ever the participant feels they can afford.

Where:
Cases of Boost Plus are dispensed through the Senior Services
Administration Office, located in the Bellingham Senior Activity Center.
Office hours:
Monday - Friday
8:00 am to Noon
1:00 pm to 4:00 pm.
Delivery is available to those individuals who are homebound. |
